A MOV from an iPhone, a Mac screen recording or QuickTime holds HEVC or H.264 video and AAC sound. An MP4 can hold exactly the same streams, so the page moves them into the new container without decoding a single frame. The MP4 comes out almost the same size as the MOV, which is how you can tell nothing was re-encoded.
Sending it to someone on Windows
iPhones record in HEVC by default. It plays on Apple devices and most recent PCs, but some Windows computers need an add-on for it. If the person you are sending it to cannot play it, open More options and choose H.264: the video is then encoded again, which takes longer, and plays on practically everything.
